Care Coordination Services
At Guthrie County Hospital, our Care Coordination Services team supports patients, families, and community members by helping them navigate available health care resources. We work to ensure you receive the right services at the right time, while reducing barriers that may impact your social, financial, or medical well-being.
Our services include, but are not limited to:
- Advanced care planning
- Guidance and support with Medicaid resources
- Referrals for nursing homes, assisted living facilities, and hospice care
- Connections with the Department of Human Services (DHS)
- Assistance with a wide range of community and support services
Our goal is to help you make informed decisions and improve overall quality of life. All information, referrals, and support provided by our care coordinator are free of charge for patients, families, and community members.
